The Safe Body Art Program provides minimum statewide standards for practitioners and facilities in the business of tattooing, body piercing, permanent cosmetics and branding.  The requirements are intended to  protect both the practitioner and the client from transimission of infectious diseases through the application of proper body art procedures and the control of cross-contamination of instruments and supplies.

      Body Art Practitioners

          Requirements for Body Art Practitioner:

    • All Body Art Practioners must be over 18 years of age.
    • Submit a Copy of Driver's License or Government IDCopy of Driver's License or Government ID
    • Hepatitis B Vaccination Documentation (1 of the following):   

                             - Certification of Completed Vaccination         

                          - Laboratory Evidence of Immunity           

                          - Vaccination Declination


        Body Art Facilities

            Requirements for opening a Body Art Facility, please complete the following: 

                   Plan Check List

              Plan Review Application

              Permit to Operate Application

              Infection Prevention and Control Plan
