Assertive Community Treatment provides 24 hour intensive service coordination for consumers with serious mental illnesses such as Schizophrenia or Bipolar Disorder. Consumers referred to these programs display functional impairments in daily living skills, high risk behaviors, and a consequent high probability of hospitalization or grave disability on a regular basis. Consumers receive daily individualized service coordination. Services are provided in the filed, are strength based, and employ team caseloads with staff to consumer ratios of 1:12. Supportive Independent Living homes, whereby clients share an apartment or home with other clients and jointly share chores, cooking and utilities, are also available on a limited basis in these programs.
These services are delivered through the County Clinic, known as New Directions
as well as through an organizational provider Turning Point
. Both programs are located at the Crown Point facility. Turning Point provides a more intensive level of these services after hours and during times the County Clinic is closed.
