The Nevada County Behavioral Health leads a group of community providers who meet regularly to plan and implement policies and strategies to expand and improve integrated health care. The team includes two substance abuse providers, two primary care clinics, an acute care hospital and Behavioral Health. Support and guidance are provided from the California Institute of Mental Health and IDEA Consulting.
Current funding for this project includes Mental Health Services Act Innovation, a HRSA Grant (PDF), and Medi-Cal revenues for direct services whenever possible.
Some key accomplishments include progress toward a universal Release of Information Form and planning for primary care services provided at the Behavioral Health clinic site.
